My question would be. Did maintenance do an engine run after work was completed. Did the pilots do a proper run up on an airplane coming out of maintenance or even a first flight of the day run up. Seems if something was not hooked back up properly it should have been caught before leaving good hope...

I was told some long time ago that the term “artificial horizon” was not correct as you can’t see yaw hence forth call it an “Attitude indicator” seemed to make sense to me.

If there was water in the fuel plus the atmosphere was right for ice sounds like the perfect storm for carb ice.

It depends where the student might end up. If a young pilot finds her/him self up north in a single Cessna now flying in 3 miles and clear of cloud or SVFR and has never been flying in those conditions the learning curve and stress is very high! I did not look at the area around but if the field was full of kids playing do you think he would have landed there?

As for the AC crew that erased the CVR that should be cause for termination. If things went down like they said why did you erase it? It would prove you were right or full of shit.

Fascinating story! My biological grandfather was killed in a lightning with spartan air service in what would have been the late 50s out of CYOW on a maintenance flight. Believe it was an engine failure on takeoff.
