This is a US certificate issued on the basis of your Canadian PPL, and remains valid so long as you maintain your Canadian PPL + medical. Process to get it is a paperwork exercise (but will take a few months)

Personally I would interpret it to mean that the applicant can count the instrument rating as 20 hours of instrument time towards all applicable requirements under 421.30(4). Correct on both. See the last page of the booklet for abbreviations (combine "SME", "L", and "S").
