Considering this flight was last one for the boys/girls with an easy laid back approach. Bet they started early in day to get to Toronto then fly across country. I'd say fatigued is a BIG factor on this one.

MU II. Worst ice magnet I've flown. Picked up ice on nose and prop spinners and made cup like shapes. Slowed aircraft down quickly and unable to maintain altitude many times

All twin turbine under 12,575 lbs. do not require a POC unless you wish to do RNAV approaches, RVSM and any other items that require OPs specs.

One other item that a lot of new pilots forget is that for example: 120 hours in 30 days doesn't mean 30 calendar days like from April 1st to April 30th. Means 30 days from March 9th to April 8th, then March 10 to April 9 and so on. Need a program or make an excel program to keep track. In this case...

Its not TC per say...its CTA pullling the license for the OC's...if said company is not paying security fees to CTA or not having proper paper work for sub charter between companies or even not displaying tariffs and such in the office could be violation and have the license pulled for 30 days.........

Actually, for commercial operations, you are not allowed to fly in RVSM airspace without TCAS II after July 1/09 (in canada). You can fly in RVSM airspace with TCAS I privately. This is all good stuff but as the example above....15hrs or more a week equals 800hrs or more closer to overhaul. My previous company proved go fast...may save money short term but save on long term on overhauls costs to components and cycles...
