Departed the side of the runway on takeoff at night during with snow falling. No centerline markings were visible. The aircraft lined up a few degrees off heading and continued like that all the way to the runway edge.

I was surprised to see that it was a really high time captain and was wondering how it could happen as he must have done this kind of takeoff so many times before. It appears that he was distracted by paying too much attention to what the FO was doing instead of looking outside properly.

What was even more strange was that when he decided to reject(as he neared the left edge of the runway), his hand was not on the throttles and accidentally knocked the right throttle forward and retarded only the left one when he moved his hand to grab them....bad news.

The report doesn't really say why his hand wasn't on the throttle but one might suspect that he was reaching over to do something and looking inside.

We had a jet almost crash on a go-around when the captain was fiddling inside soon after liftoff to cancel a warning due to his mis-positioning throttles and almost descended into the water. Probably best to concentrate on piloting the aircraft at critical points in flight.
