Taking off with a concrete block that could fall off and kill someone, let alone crash the airplane:

This happened to an Air Georgian caravan crew once but the concrete block fell off and was never found

Reminds me of the guys who spun around a turboprop on an icestrip. No damage, no talk about it....until along time later at a party. Then it got back to the boss.
