I think keeping the mic down is a rare occurence. Someone might do this mistake but is rare. Normally many pilots remember to lift the thumb

Actually you'd be surprised, though it's more common with the velcroed PTT where one might trigger it with one of their other fingers when they're white knuckling it around the circuit.
