On 21 May 2021, only 2 days after Annual Inspection completed, Piper PA31 Navajo Crashes Shortly After Take Off from Myrtle Beach Airport,(KMYR) South Carolina.

NTSB’s Preliminary Accident Report:

NTSB discovered that the elevator trim tab was installed upside down and reversed after being fully disassembled during the course of Aircraft’s Annual Inspection which was completed only 2 days prior to the accident. If true, that would cause the aircraft to do the exact opposite of what the pilot expected from that critical system. Among other things, the aircrafts handling characteristics would likely cause confusion for the best of pilots.


The airplane was piloted by an experienced American Airlines Airbus Captain. The pilot attempted to circle and land in the departure airport, but crashed 2.5 miles Northwest of the airport. According to Flight Aware, the aircraft was airborne approximately 2 minutes,

The pilot, who was the sole occupant of the aircraft was fatally injured during the impact and the post impact fire.

This is not the first and won’t be last. Be very suspicious of an aircraft which recently had maintenance done.

Trim tab operation should be checked for proper direction of operation before the first flight of the day in any small aircraft that one is not extremely familiar with(such as rentals). The Navajo is a bigger aircraft and more difficult to do this, but it is imperative on aircraft of any size to check for proper direction of tab movement after any maintenance where it could have been affected.
