AgPlane Incident thread

Oh oh.....The first one is not even an airplane......

C-FBWE, a Hiller UH-12E helicopter operated by Western Aerial Applications Ltd., departed
Winnipeg City Heliport (CWG2), MB to apply control pellets to a farmers field. While seeding and
making a tight turn, the helicopter was unable to maintain altitude and the pilot attempted a forced
landing. The pilot made an autorotation at 20 feet above the ground and landed hard; making
contact with small tree branches and a pole guy wire. The helicopter sustained substantial damage
to its skids. The pilot was not injured and the 406 ELT activated.

C-FELO, a Thrush S2R-T34, operated by Provincial Airways, was conducting an aerial application
flight near Glenavon, SK, located 15 miles south west of Grenfell (CKU6), SK. While applying
chemical onto a canola field, the aircraft’s vertical fin struck a powerline. The pilot then carried out
a hard landing straight ahead and the aircraft came to a stop in an upright position. The pilot was
not injured and the aircraft sustained substantial damage.

C-FOBO, a Robinson R44 helicopter operated by Hanley Heli Service, was conducting an aerial
application flight near Bradwell Saskatchewan, MB. The helicopter departed from a road to spray
an adjacent wheat field, loaded with 30 gallons of chemical. During the initial application pass, in a
westerly direction, the helicopter struck a powerline and the pilot conducted a hard landing straight
ahead. The pilot was not injured; however, the helicopter sustained substantial damage.

C-GICZ, an Air Tractor AT-502B operated by Arty's Air service, was conducting an aerial
application flight near Ste. Agathe, MB. While applying chemical on a canola field, the aircraft's
rudder struck a powerline. The pilot then carried out a low level recovery straight ahead over the
town of Ste. Agathe and diverted to Winkler (CKZ7), MB where the aircraft landed safely. The pilot
was not injured and the aircraft sustained damage.

The operator reported that the right wing tip, the upper rudder cap, propeller blade, and pump
blade fans were damaged and will be replaced prior to returning to service.

C-FJVL, an Arty's Air Service Ltd. Air Tractor AT-502B, was conducting agricultural application
operations 29 NM SSE of Camrose (CEQ3), AB with 1 pilot on board. During an application pass
the aircraft collided with terrain; the pilot was fatally injured. There was no post-impact fire and the
ELT did transmit. The Edmonton TSB office deployed 2 investigators to the accident site.

The 41-9 Aerial Ltd Robinson R44II, C-GGJS (S/N 12254), was carrying out spray operations on a
canola field 18NM north of Boyle (CFM7), AB when on the last tank of the day, during one of the
final passes, the helicopter came in contact with the canola crop. Control was lost and the
helicopter tumbled forward, coming to rest inverted in the crop. The pilot was uninjured and was
able to extract self from the wreckage. The helicopter was substantially damaged. There was no
fire. The ELT did not activate.

C-FCRF, an Air Tractor AT-502B operated by Prairie Sky Crop Solutions, was conducting an aerial
application flight near Niverville, MB. While applying pesticide on a crop, the aircraft's propeller
blade and tail fin came into contact with a powerline. The pilot carried out a low-level recovery,
jettison the remaining pesticide load and returned to Niverville, MB where the aircraft landed safely.
The pilot was not injured and the aircraft sustained damage. Part of the tail fin and rudder
remained lodged in the powerline until it was removed by Manitoba Hydro.

The operator reported that the propeller blade, tail fin and tail rudder was to be repaired prior to
returning the aircraft to service.

C-FIAM, a Call Aircraft S-1B1X, was operating from a farm air strip approximately 15 nm N of
Coronation (CYCT), AB. Check out instruction was being provided to a licensed pilot by an
instructor. Upon touchdown the aircraft bounced and control was lost. The aircraft was landing
west to east and departed the right side of the runway, contacted some trees and then came to
rest. The occupants were uninjured and were able to exit the aircraft unassisted. The aircraft was
substantially damaged.

C-FCBK, a Thrush S2R-T34 operated by Kindersley Airspray, was performing aerial application of
a field 7NM WNW of Luseland (CJR2), SK. During a turn at a low altitude, the aircraft's left wing
contacted the ground. The aircraft subsequently collided with the terrain and it was destroyed. The
pilot was injured and airlifted to hospital.

C-GDKO, an Air Tractor AT-402B operated by Western Canadian Aerial, was conducting an aerial
application flight from Gilbert Plains Airport (CJH2), MB. While on an application pass
approximately 10nm to the northeast of CJH2, the aircraft's landing gear collided with a power line.
The wire cutters on the gear legs severed the line. The pilot stopped spraying and returned
uneventfully to CJH2. The pilot was uninjured. The aircraft sustained minor damage, limited to
spray nozzles and the pump fan blades.

C-FMXB, a Thrush S2R-H80 aircraft operated by Agmax Ltd., was performing an aerial application
flight 5.5 nm NE of Minnedosa (CJU5), MB, near the community of Polonia, MB. During a low
pass, the aircraft inadvertently touched the crop and the landing gear was ripped from the
fuselage. The aircraft subsequently belly landed onto the ground in an upright position. The pilot
was not injured. The aircraft sustained substantial damage to the landing gear, lower fuselage,
engine, and propeller.
