Has anyone encountered this? I still haven't received my booklet despite it being allegedly printed almost 3 months ago. Further, I fail to understand how the booklet can be printed without a booklet number.

The issue date is the date when the button is hit to issue in the computer system, the print date could be months later

Same here. I called them and they sent me a blue paper extension for 3 months.
