As to no missed approaches, you should read up on Apollo 10. The entire mission was as missed approach.

If you knew where the missions took place you would realize most of the missions landed on the near side relatively close to the centre of what you can see from the Earth. In that picture the Earth is above their heads. I believe 15 or 17 can't remember which was the only one to have the Earth low in the sky.
