When I lived in Montreal a couple of years ago, I found that the FAA Medical Examiners charged outrageous fees. Another option-- drive down to Plattsburgh. Enough money left over for a nice dinner and then some.

FAA Doctor AME in Syracuse NY
Just completed a 1st Class medical with Dr. Wolf in Syracuse whose staff
faxed off a signed copy of the medical and EKG to TC in Toronto.
All I have to do is send via snail mail the original copy signed by the doctor.
There is however a new form on TC.ca web site for foreign AME's to complete
however I did not know about it until after it was faxed.
To TC's credit, they have significant discretion with foreign issued Medical certificates
that is not available in other countries such as Australia or Europe.
8 hours of driving 600K and $179 later, it was done. Probably not a lot of difference
in going to Montreal but I chose Syracuse NY as it suited me.

Dr Frechette of Hawksbury, Ont. does NOT appear on
the FAA List of AME's in Canada.
I called the two in Montreal and could not get an appointment
when I needed it but could in Syracuse which is not much further for me.
Both charge more than the rate in the US as well.

Re: FAA Doctor
There have been a number of changes that affect Canadians who wish to
do an FAA medical and then have Transport Canada recognize that medical.
First, the FAA doctor is now prohibited from providing a signed copy of his form to the pilot.
Second, you need to obtain three items.
A, you need to send TC.ca a copy of the EKG and 1st class medical certificate,
signed by the Doctor, HARD COPY, to TC.ca
B. You need to have the FAA send a COPY Of their form to TC.CA via FAX.
That's it. But allow 4 working days for the medical to reach FAA from the doctor
and another week or so for TC to issue the sticker and invoice you that fee
for which they have no apparent costs.
It's almost impossible getting an appointment when you need it that means
you end up getting it early and only getting five months out of six month medical.
The fun and games of have two licenses.

Re: FAA Doctor in Montreal?
The OK city clowns are still screwing with me ONE YEAR LATER...
They need this or that, but they dont tell you right away,
they claim for three or four months they have not received anything,
then they finally admit they had the report all along but need these things,
then, when you comply, the comedy start again, denial, etc...Obama's world for you!
The Soviet Kafka experience! But I gave up on the US...I retired!
WHAT DIFFERENCE DOES IT MAKE?

Re: FAA Doctor in Montreal?
Dr Robert Perlman:
TC medical: 300CAD
FAA medical: 550CAD
TC and FAA: 650CAD
The aviation medical examiners in Montreal do fleece you! I paid only 150CAD for my TC renewal in Toronto last year, here it is simply double.

Re: FAA Doctor in Montreal?
Perlman is too expensive. Anne Thériault charges significantly less.
