Pilot reported as 54 year old Montrealer Jean-Luc Deba, and helicopter reported as 1975 Bell 212 C-FJUR

CADORS 2011C1660

C-FJUR, a Bell 212 operated by Campbell Helicopters, was bucketing water in firefighting activities when it crashed into Slave Lake about 10 miles northwest of the community of Slave Lake. Emergency services attended the site and pulled the pilot out of the wreckage which was in about four feet of water. The pilot received fatal injuries in the accident. TSB Edmonton sent two investigators to the scene. A Minister's Observer has been appointed to assist in the TSB Class 3 investigation.

Just some info I heard today from another helo pilot who was working the same area. He told me that this crash was caused because the pilot caught his sling load in the trees along the shore when he was retrievng water. Although he was a high hour pilot he did not have many carrying a sling.
