$300, ish. A bit pricey but my CAME is a 10 minute walk, across the street from my house, so I let it slide on account of how convenient his office is.

I think I paid $150. Which on the one hand is good, but on the other hand, since it involved nothing but a 15 minute phone call with the CAME, is not an unhealthy hourly rate. But on the third hand, look at all the time I saved not having to travel to a medical office.
